Medicare Advantage Bathroom Safety Benefits: What Plans Can Cover

Since 2019, Medicare Advantage plans have been allowed to offer bathroom safety devices as an extra benefit. Here is what that usually means in practice, why it rarely covers a whole walk in tub, and how to find out what your plan pays.

Short answer

Does Medicare Advantage pay for bathroom safety?

Some plans do, for devices. CMS lets Medicare Advantage plans offer home and bathroom safety devices, like grab bars, shower seats and handheld showers, as a supplemental benefit, sometimes with installation. The 2018 guidance left out capital or structural changes to the home, so most plans will not pay for a walk in tub or a full shower conversion. Benefits vary by plan and year.

Ask your plan these 4 questions

  1. Do you have a home or bathroom safety benefit this year?
  2. Which items, and is installation included?
  3. Is there a yearly dollar limit, and do I need an order from my doctor?
  4. Do I have to use your vendor?

Get the answer in writing, or find it in your plan's Evidence of Coverage.

What changed in 2019

Medicare Advantage plans can offer extra (supplemental) benefits that Original Medicare does not, as long as they are "primarily health related." In April 2018, CMS widened that definition for plan year 2019 to include items that help prevent injuries or make up for physical impairments.

One of the new categories was home and bathroom safety devices and modifications: non-Medicare-covered safety devices to prevent injuries in the home or bathroom, with the exception of capital or structural improvements to the home. Examples plans commonly list include shower stools, handheld showers, grab bars, bathroom rails and raised toilet seats.

Why the walk in tub usually isn't covered

Replacing a bathtub with a walk in tub or a roll in shower means removing a fixture, changing plumbing and sealing new walls. That is a structural improvement, which the CMS guidance excluded from this benefit. So the realistic expectation is:

  • Often possible: grab bars, a shower chair or tub bench, a handheld shower, a raised toilet seat, a home safety check.
  • Rarely: a full walk in tub or roll in shower installation.
  • Never: Original Medicare paying for a walk in tub. It treats tubs as a convenience item. Details here.

Plans for people with chronic illness can also offer special extra benefits, which CMS calls SSBCI. If your parent has a chronic condition, ask the plan whether any home modification benefit applies to them. See all options: 8 programs that pay.

How to use the benefit well

  1. Call the member number on the card and ask the four questions above.
  2. If devices are covered, get them now. Grab bars and a shower seat make the current tub safer while you plan the bigger change.
  3. Ask the doctor for a note on the medical need. It helps with the plan, a VA or Medicaid application, and the tax deduction.
  4. Get a written, itemized quote for the walk in tub or roll in shower so every program sees the same numbers.

Our price, for planning

A fully installed walk in tub or 60 x 30 roll in shower from us is about $15,500 after the current $2,000 promotion, or $15,250 for veterans. Oversized one-piece showers run about $17,000, and wide or transfer tubs start from about $19,000. Quotes are written and itemized, by phone or from photos, and we do not do in-home sales visits.

Watch out for anyone who says Medicare will "cover the whole tub." Original Medicare will not, and an Advantage plan almost never pays for the installation.

Questions

Medicare Advantage Bathroom Benefits FAQ

Does Medicare Advantage cover walk in tubs?

Rarely. CMS lets plans offer bathroom safety devices as an extra benefit, but its 2018 guidance excluded capital or structural home improvements, and installing a walk in tub is one. Check your plan's Evidence of Coverage.

What bathroom items can Medicare Advantage cover?

Plans that offer the benefit commonly cover items like grab bars, shower stools or chairs, handheld showers, bathroom rails and raised toilet seats, sometimes with installation.

How do I find out if my plan has a bathroom safety benefit?

Call the member services number on your card or read the supplemental benefits section of your Evidence of Coverage. Ask about item limits, dollar caps, vendors and whether a doctor's order is needed.

Does Original Medicare pay for grab bars or shower chairs?

Generally no. Original Medicare treats most bathroom safety items as convenience items, not durable medical equipment.

What pays for a walk in tub if Medicare doesn't?

VA HISA grants for veterans, Medicaid home modification waivers in many states, the medical expense tax deduction for those who itemize, and financing.
